The High School clinic is staffed by Nurse Underwood. The clinic is open Monday through Friday 7:30 am to 11:40 am and 12:10 to 3 pm.

Clinic - Medical History & Physical Form

This form should be used for all physicals (athletic and non-athletic activities).

The history portion of this form should be completed by a parent/guardian and the physical portion of the form needs to be completed by the student's physician during their physical.
